Simple Data Entry Form Access

In the database window select the table for data entry.

Enter the data inputs in the form. Go to the buttons section in the app. To create a form from a table or query in your database in the navigation pane click the table or query that contains the data for your form and on the create tab click form. Start the form wizard click the create tab on the ribbon and click the form wizard button.
